Effective date: September 15, 2026
Moderation Medical, LLC (“Moderation Medical,” “we,” “us,” or “our”) operates the website at moderationmedical.com (the “Site”). This policy explains what information the Site collects, how it is used, and what choices you have.
What this policy covers — and what it does not
This policy applies only to your use of this Site.
It does not cover protected health information (“PHI”) created or maintained in connection with treatment. If you become a patient of Moderation Medical, your health information is governed by the Health Insurance Portability and Accountability Act (“HIPAA”), by 42 C.F.R. Part 2 where it applies to substance use disorder treatment records, and by our Notice of Privacy Practices, which is provided separately.
This policy also does not cover websites operated by others. Our appointment and patient portal links take you to IntakeQ/PracticeQ, a separate platform with its own privacy practices. Once you leave this Site, this policy no longer applies.
Information we collect
Information you give us. The Site has no contact forms, no newsletter signup, no account creation, and no chat feature. We do not collect names, email addresses, phone numbers, health information, or payment information through this Site.
If you email us at the address listed on the Site, we receive whatever you choose to include in that message. Please do not send sensitive health information by email — email is not a secure channel.
Information collected automatically. Like most websites, this Site collects limited technical information when you visit:
- Analytics. We use Google Analytics 4 to understand how visitors use the Site. Google Analytics sets cookies and assigns a pseudonymous identifier to your browser. It collects information such as pages viewed, time on page, approximate geographic location derived from your IP address, referring website, and general device and browser characteristics. Your IP address is transmitted to Google as part of this process. We have not enabled Google Signals, so we do not permit Google to associate your activity on this Site with a signed-in Google account for our purposes. Google’s handling of this data is governed by Google’s own privacy policy.
- Server logs. Our hosting provider maintains standard server logs, which typically record IP addresses, timestamps, requested pages, and browser type. These are used for security and operational purposes.
- Content delivery networks. The Site loads certain resources, such as icons and fonts, from third-party content delivery networks. When your browser requests those files, the CDN provider receives your IP address and basic request information. We do not control and do not receive that data.
Cookies
The Site uses cookies set by Google Analytics as described above. We do not use advertising cookies, and we do not run retargeting or behavioral advertising of any kind.
Most browsers let you block or delete cookies through their settings. Google also offers a browser add-on that opts you out of Google Analytics across all sites. Blocking cookies will not affect your ability to use this Site.
How we use this information
We use the limited information described above to:
- Understand which pages visitors find useful and improve the Site’s content and structure
- Maintain the security and reliability of the Site
- Respond to messages you send us
What we do not do
- We do not sell your personal information.
- We do not share it with advertisers or data brokers.
- We do not use it for targeted advertising.
- We do not combine Site analytics data with patient records.
- We do not knowingly attempt to identify individual visitors from analytics data.
Your choices
You can control cookies through your browser settings or install Google’s opt-out browser add-on. You can also visit the Site using your browser’s private browsing mode.
Our Site does not currently respond to Do Not Track signals, as no common standard for them has been adopted.
Colorado residents
The Colorado Privacy Act gives Colorado residents certain rights regarding personal data, including rights of access, correction, deletion, and portability, and the right to opt out of targeted advertising and the sale of personal data. The Act’s obligations apply to businesses meeting specified processing thresholds, and information governed by HIPAA is exempt from its requirements.
Regardless of whether those thresholds apply to us, you are welcome to contact us at the address below with any request or question about the information described in this policy, and we will respond.
Children’s privacy
Our services are for adults 18 and older. This Site is not directed to children under 13, and we do not knowingly collect personal information from children.
Security
We take reasonable measures to protect the Site. No website or internet transmission is completely secure, and we cannot guarantee absolute security. Again, please do not send health information or other sensitive details by email.
Changes to this policy
We may update this policy from time to time. When we do, we will revise the effective date at the top of this page. Material changes will be reflected here, and your continued use of the Site after an update constitutes acceptance of the revised policy.
