- You must be 18+ for treatment.
- You must be currently living in Colorado.
- Your provider reserves the right to refer to a higher level of care if deemed medically and psychiatrically necessary.
- At this time we do not prescribe controlled substances such as suboxone or benzodiazepines.
- Treatment includes an hour long intake evaluation and 30 minute follow up visits as determined by your provider.
- If opting for self pay, the hour long intake evaluation is typically $350 and 30 minute follow up visits are typically $160.
- Self payments and copays are due the day of service.
- After booking an initial appointment, intake forms will be emailed to you. Please complete the emailed forms at least 24 hours prior to the appointment. If these forms are not completed, you will be asked to reschedule.
- You may reschedule or cancel your appointment via the client portal at least 24 hours in advance. Late cancellations will be charged a $100 dollar fee to your card on file.
- If you are more than 10 minutes late to your appointment, you will be charged a $100 late fee to your card on file.
